The title Our Lady of Grace and the image on the Miraculous Medal are one and the same. On November 27, 1830, the Blessed Virgin Mary appeared to Saint Catherine Labouré, a young Daughter of Charity novice in Paris, and showed her this precise image: arms extended, rays of light streaming from her rings toward the earth below — representing the graces she obtains for those who ask. She asked that a medal be struck with this image, promising: "All who wear it will receive great graces, especially if they wear it around the neck."
The outstretched arms with downturned palms are the defining gesture of this image — Our Lady actively pouring grace downward upon the world, upon us. It is not a passive posture.
